CCN's February seminar will be a presentation on at-home care titled "Synergy HomeCare Through PocketRN".  Pocket RN is a new coordinated care program that supports family, friends and caretakers.  We will also hear about the basics of Synergy HomeCare including care cost information, a customized care plan and a free home assessment.  A free lunch will be served at 11:30 am and the presentation will run from 12:00 to 1:00 pm.  This is a free event and all are welcome.  Please register by Sunday February 15.
